EECP: The Non-Surgical Cardiac Therapy Most Cardiologists Won't Tell You About

Enhanced External Counterpulsation has a 35-year safety record and FDA approval for angina — yet remains underutilised. Here's what the clinical evidence shows, and who benefits most.

What Is EECP?

Enhanced External Counterpulsation (EECP) is an FDA-approved, non-invasive cardiac therapy that uses pneumatic cuffs wrapped around the calves, thighs, and buttocks. These cuffs inflate and deflate in synchrony with the heartbeat, creating a counterpulsation effect that increases blood flow to the coronary arteries.

The Clinical Evidence

EECP has accumulated over 35 years of clinical evidence and is FDA-approved for chronic angina and congestive heart failure. A 2023 meta-analysis of 13 randomised controlled trials confirmed significant improvements in exercise tolerance, angina frequency, and quality of life compared to control groups.

Who Benefits Most from EECP?

EECP is particularly valuable for patients with: refractory angina (chest pain despite medication), post-bypass or post-stent patients with continuing symptoms, heart failure with reduced ejection fraction, and patients who are not candidates for surgical intervention.

The Standard Protocol

The standard EECP course consists of 35 one-hour sessions over 7 weeks. Most patients begin experiencing symptom relief within the first 15–20 sessions. The hemodynamic improvements — including formation of new collateral blood vessels — persist for 3–5 years after completion.
